How to submit a claim:
The easiest way to submit a claim is to “add an expense” in the Melody Benefits app. (Links to download the app are further below.)
To submit a claim manually:
Get a claims form. You can download the form with this link. You can also request the form through email ([email protected]), WhatsApp, or text. Forms are available in many languages on our website or by request.
Fill out the form.
Send us the completed form by email ([email protected]) or fax (781-693-9531).
